Name of Practicum Site, Unit or Program:
Crossroads Refugee Health Clinic, Women’s College Hospital
Location: 76 Grenville Street, Toronto, ON M5S 1B2
Description of the practicum site and student activities:
The Crossroads Refugee Health Clinic at Women’s College Hospital provides comprehensive primary health services to newly arrived refugees during their first two years in Toronto. Our team consists of medical staff, nursing, medical administrators, social work, psychiatry as well as learners and volunteers. The majority of the clinic’s clients are refugee claimants fleeing persecution, many of whom are living in Toronto’s emergency shelter system. The Crossroads Clinic’s client population includes adults, families with children, and unaccompanied youth. Social work services offered include psychosocial assessment, mental health counseling, information and referrals, case management, and advocacy. Social work services focus on addressing trauma, adjustment, and mitigating barriers to accessing services in the community. Clinic staff make frequent use of language interpretation for all service delivery.
