Description of the practicum site and student activities:
Practicum Description
Large, downtown, hospital-affiliated academic family health team. Diverse population reflecting population of inner-city as well as commuters and government workers whose family physician is located at our practice. Primarily providing short term individual and group counselling and psychotherapy for a broad range of issues including depression, anxiety, relationships, work stress, grief, trauma, adjusting to a new diagnosis. Will also include assisting clients needing to access resources, referrals in the community.
Student activities to develop social work competencies
Direct Service Delivery/Interventions/Working with Recipients of Service:
- Conduct intake/needs assessments
- Provide resource and referral information
- Develop, promote, plan and/or (co)facilitate educational groups or workshops when schedule permits
- Carry individual case-load providing individual assessment, counselling, intervention and evaluation of interventions
- Collaborate with interprofessional team
- Apply best practices as outlined by the organization
- Undertake instrumental activities that support the work with recipients (e.g., assistance forms, benefit forms, referrals, goals for therapy)
Interprofessional collaboration:
- Present cases at case conferences
- Attend staff/administrative meetings/rounds
Indirect Service Delivery:
- Prepare and maintain case notes, reports, statistics which meet the organization’s accountability guidelines
- Collect and/or analyze data
- Contribute to research and other organizational initiatives
- Develop skills in cross-cultural/spiritual competence
- Research and add to inventory of resources relevant to client populations
