Year 1 Practicum

Name of Practicum Site, Unit or Program: Upper Grand Family Health Team – Mental Health Team

Location: 6420 Beatty Line Fergus, ON

Description of the practicum site and student activities: Providing counselling for clients of the FHT, working with the primary care physicians and other allied health staff.  The mental health team works with individuals, generally 18+ with mild to moderate mental health challenges.  There is an opportunity to also work with the palliative team with their caregiver support group.

Other requirements:

Does your practicum require any previous experience? Experience working with people who struggle with their mental health is required and some basic counselling skills is also required.  Documentation skills are also required as well as a comfort level in using documentation software.

Is this practicum in-person, remote or hybrid? Hybrid

Will this practicum involve direct work or a mix of direct and indirect? This is primarily direct social work, with opportunities to help with the Quality Committee on more macro level social work

  • Are there any other accessibility considerations that potential applicants with disabilities should be aware of? The office is on the ground floor, there is an elevator if there is a need to go to the second floor and most doors are accessible.
