Description of the practicum site and student activities:
Two Practicum Opportunities at THP
Practicum Opportunity 1 – Child and Adolescent Mental Health Services:
Trillium Health Partners Child and Adolescent Mental Health Services provides outpatient services for children and adolescents up to age 18, and their families, who live in Mississauga and south Toronto. Treatment may include individual or group therapy with a focus on CBT/DBT skills.
Practicum Opportunity 2– Renal Care Centre:
Trillium Health Partners Regional Renal Program provides care to patients with chronic kidney disease.
The Renal Program consists of in-centre hemodialysis, peritoneal dialysis, home hemodialysis and MCKC (Multi-care Kidney Clinic). Patients are followed by the interprofessional team which includes
Nephrologist, Nurse, Dietitian, Pharmacist, Social Worker, Vascular Access RN and Nurse Navigator.
The approach is Person Centred Care, providing education and support to patients and caregivers.
Student activities: learners will have the opportunity to:
1) shadow members of multi-disciplinary team to understand their roles and responsibilities;
2) participate in team meetings and/or case conferences;
3) engage in conducting psychosocial assessments, connecting patients/families with community resources
4) participate in Decentralized Social Work Practice Council meetings and professional development meetings
5) Participate in CANSW (Canadian Association of Nephrology Social Workers) monthly Webinars
6) Clinics hours are 8 am to 4 pm; occasional evening may be required.
