Description of the practicum site and student activities:

The Family Health Team is a multidisciplinary team where social work students provide short term counselling and case management to patients. There is also opportunity for outreach support, including home visits or community accompaniments. Case management includes connecting patients with resources for income, housing, employment, legal support, etc.
