Southlake Health
Year 2 Practicum
September 2025- April 2026
Name of Practicum Site, Unit or Program: Outpatient Neuro Rehab
Location: Southlake Health (Formerly Southlake Regional Health Centre)
596 Davis Drive, Newmarket, ON L3Y 2P9
Description of the practicum site and student activities:
The Outpatient Neuro Rehab is a multi-disciplinary allied health OHIP-funded program for patients 18+ who have had a one-time neurological event, such as stroke, TBI, GBS or MS, who are now medically stable and require rehabilitation services (please note we do not offer services to patients with long-term/ongoing neurological or neurodegenerative diseases, such as dementia or Parkinson’s). The majority of patients seen are 65+, however, young adults/adults are also seen regularly.
Students will have the opportunity to shadow, co-lead and individually lead psychotherapy sessions for patients and/or their caregivers, co-facilitate support groups and/or patient-family meetings, screen patients for mental health using the Canada Stroke Best Practice-approved validated measures, and possibly shadow/co-lead sessions with interdisciplinary professionals (such as PT, OT, and/or SLP), including documenting/charting duties. Additional student activities include indirect services such as resource gathering and making referrals/community connections, however, this practicum is direct-service heavy. The setting is fairly fast-paced but there is opportunity for students to develop and implement services based on their area of interest, within the scope of our program / service user population needs. There are no evening or weekend hours.

- Is the practicum considered fast-paced? Fast
- Is there a high noise level in the practicum? Medium
- Is the site wheelchair accessible? Yes
- What are the physical demands of the practicum? None
- Are there any other accessibility considerations that potential applicants with disabilities should be aware of? Space is limited, our department can become crowded, mildly noisy, and/or we often have to share office rooms.
We are about a 10-15 minute walk from the Newmarket Go station. There are bus stops through York Region Transit right by the hospital. If not taking public transit, parking is paid and about a 3 minute walk from the hospital. Our department is on the main floor of the hospital.
