Description of the practicum site and student activities:
The student will be supervised, while supporting patients on the acute, birthing and ICU units.
The student will be making referrals to community partners.
The student will provide advocacy, on behalf of the patients.
The student will provide supportive counselling.
The student will connect patients to community organizations.
The student will document on each patient, where contact has been made.
The student will support patients with navigating challenging bureaucratic systems.
The student will support patients with completing complex government forms.
The student will be attending interdisciplinary rounds on a daily basis.
The student will attend social work meetings.
