Description of the practicum site and student activities:

SLE in Social Determinants of Health description:

Social Work students will be placed in an innovative, student-led environment (SLE) within Holland Bloorview Kids Rehabilitation Hospital’s Social Determinants of Health strategy.   In this placement, students will contribute to the Family Navigation Hub and develop their competencies for practice by supporting families who have been identified in needing support related to their Social Determinants of Health.  This fully immersive placement will appeal to 2nd year Social Work students who are highly motivated, self-directed learners, and have strong interpersonal and communication skills. Students will be actively engaged in an environment that facilitates a family’s ability to enhance their capacity to access available services by coaching clients/families in system navigation, participating in community outreach activities in partnership with community agencies and by contributing to research and program development.  Placement will be a combination of indirect and direct work with clients. The placement will be in person but continue to provide a hybrid model of care to clients including virtual sessions. Successful candidate must be willing and able to come on site and follow all health and safety protocols.

SLE in Social Determinants of Health competencies:

  • Develops effective coaching and communication skills to empower clients with the skills that they need to succeed
  • Develops effective independent time management and prioritization skills
  • Collaborates effectively with the inter-professional team to provide high-quality, holistic care and services focused on empowering clients and families with the skills they need to meet their goals
  • Forms therapeutic relationships with clients and uses self as an agent of change with clients to meet their goals
  • Sets collaborative and client-centered SMART goals as applicable
  • Facilitates fostering health promotion, community awareness, and educational initiatives (the pillars of wellness) with clients and families
  • Uses standardized and non-standardized assessments to assist in goal setting and measurement of success as applicable
  • Develops and practices critical thinking skills to guide assessment and intervention with clients and families
  • Engages in opportunities to become familiar with and partnering with community agencies that offer key services to clients and families
  • Develops and practices solution-focused, strengths-based, client-centered language to inform documentation and inter-professional communication
  • Actively participates in on-going program development and evaluation of the student lead environment to improve learning, access to resources, and potential projects or components that contribute to the student experience and service provision
  • Supports clients to connect and navigate to resources in the community.
  • Develop community partnerships
  • Delivery of workshops and education sessions in partnership with community partners.
  • Provide 1:1 short term consultation and navigation support with clients

Extended virtual care usage online ZOOM, staring at computer screens, loud unpredictable codes due to busy hospital setting. No designated student space so may have be comfortable utilizing various working stations, rooms and set ups.
