Description of the practicum site and student activities:
This Master of Social Work placement focuses on supporting the role of Integration Lead within the Mental Health and Addictions sector. The student will collaborate with regional Mental Health and Addiction providers, Primary Care, hospitals, Indigenous service providers, community partners, and other priority populations to enhance system integration and improve health outcomes.
Key responsibilities include:
- Assisting in the development and submission of monthly and quarterly reports for Ontario Health Teams (OHTs) and Ontario Health West.
- Supporting the BBNOHT Mental Health & Addictions Steering Committee to develop and implement strategic projects.
- Leading and contributing to quality improvement initiatives at both the OHT level and within CMHA BHN.
- Engaging in regional communication strategies and liaising with leadership to implement complex projects.
- Analyzing mental health and addiction services to identify opportunities for resource optimization to support population health management.
- Facilitating leadership in meetings to advance project goals and ensuring alignment with timelines and requirements.
This placement provides an opportunity to develop leadership, strategic planning, and quality improvement skills while contributing to impactful mental health and addiction initiatives.
