Description of the practicum site and student activities:
The practicum site is Holland Bloorview Kids Rehabilitation Hospital. This is Canada’s largest rehabilitation hospital and treatment centre for children and youth.
The placement will consist of Wednesdays and Fridays in the Student Led Environment (SLE) of the Bridging to Adulthood Program under the supervision of social worker Halla Fahmi. Thursdays will be spent with social worker and marriage and family therapist Kathy Gravel on the Brain Injury Rehab Team (BIRT).
The population served by Kathy are inpatient and outpatient children with Acquired Brain Injuries (ABI) and their families. There are opportunities to attend lunchtime education sessions and participate in committees. Services offered will include clinical assessment, intervention and consultation using outcomes-based psychotherapy and resource counselling. Kathy uses strength focused interventions including Cognitive Behavioural Therapy, Solution Focused Therapy, Narrative Therapy, Eye Movement Desensitization and Reprocessing (EMDR), Emotion Focused Therapy (EFT) and Play Therapy. Kathy has had many incredible MSW students over the years. In addition to being a Social Worker, Kathy is trained as a Registered Marriage and Family Therapist (RMFT). It is a privilege for Kathy to work and learn from MSW students in this wonderful placement opportunity.
Bridging to Adulthood Team
The SLE portion of the placement is a highly sought-after opportunity for student leadership in a unique interdisciplinary team. The team consists of social work students who work together to co-lead a large variety of “Transition Pop-Up” events in partnership with adult sector community agencies. Transition Pop-Ups are group events that help clients and families prepare for the transition to adult life through gradual introductions to new community service providers and practical support to accomplish transition-related tasks. Topics include funding, planning for life after high school, sexuality and relationships, parenting with a disability, guardianship, finding primary care, advocacy, healthy lifestyle choices, and more. The SLE team is guided by Halla Fahmi, MSW, RSW, a registered social worker who is delighted to offer opportunities for student growth in independent practice management, interdisciplinary team collaboration, group facilitation skills, forming cross-sector partnerships, program development, and leadership skills. The population for this portion of the placement includes all Holland Bloorview clients aged 14+ and their families.
