Description of the practicum site and student activities:

Social work students will have the opportunity to provide crisis intervention to clients awaiting the childminding service. Each student will be assigned their own client cases, where they will support clients from intake to discharge. Students will conduct both telephone and in-home client assessments and complete documentation for service plans. They will schedule home visit assessments to review and discuss care plans, making appropriate recommendations for all clients receiving services under charitable programs. Additionally, students will assess any further client needs and ensure that client goals are being met .

Field visit days will be scheduled for two days per week, with a target of six visits per week. Students will develop and discuss discharge plans with clients and/or referral sources, providing linkages to other community agencies or government funding sources before VHA discharge.

Students will also participate in weekly case meetings to review client cases and discuss any complex situations. All client information, including scheduling, care plans, service changes, and other relevant documentation, must be recorded and updated in the online database. Students will contribute to team meetings with child and family workers.
