Description of the practicum site and student activities:

We are a community health care agency that provides a full range of community health care and support services, making us a distinctive agency in Toronto offering a blend of community support services, community mental health and addictions services, supportive housing, interprofessional primary care and home care services. We serve a diverse client population including those with complex health care needs and those who are uninsured.

As a social work student, you will work with our Primary Health Care/Allied Health Care Area Team located at the Dundas Site. You will have the opportunity to conduct individual psychosocial assessments, provide psychotherapy and counselling, work collaboratively within the interprofessional team, and plan and facilitate group programs. This role also includes assisting with resource navigation and care coordination. This position provides the unique opportunity to observe a 21-week DBT skills group as well as assist with facilitation of the group.
