Description of the practicum site and student activities:

Across Boundaries is a Mental Health agency providing a range of support services to people from ethno-racial communities who are experiencing mental health and addiction challenges. Across Boundaries utilizes a holistic approach to serve individuals from racialized communities, with sensitivity to their needs, based on anti-racist, anti-oppressive, and anti-Black racism frameworks. The services are offered using strength-based approaches that foster a sense of individual cultural inclusion and celebration.

The Intensive Case Management Services at Across Boundaries provide services to those experiencing serious mental illness/addictions and may be involved with the criminal justice system. The students will report directly to a Team Lead and Program Manager and work collaboratively within and across teams to provide prompt and appropriate access to services to individuals from racialized communities who are experiencing mental health problems and substance use challenges. The students will provide more non-intensive services depending on skill level and user needs and will assist in Social Rehab Program.
