Description of the practicum site and student activities:

We provide individual case management services to adults living with HIV and/or who identify as transgender, and who also face barriers related to mental health, substance use and homelessness.

This practicum would involve the following activities:

-direct client support with case management needs

-referrals to external services and system navigation

-client accompaniments to appointments

-advocacy/coordination with external agencies and systems

-completion of program applications (e.g. housing applications)

-informal counselling support

Practicum Requirements:

☒ Police Reference/Vulnerable Sector Check

☒ Travel by public transportation between home visits, or satellite offices, or community meetings, etc.

☒ Comfort working with the population being served

☒ Comprehensive report writing and documentation
