Description of the practicum site and student activities:
The Social Work Team at Grandview Kids is committed to supporting the psychological, social and emotional well-being of children and youth with developmental, physical, and communication needs and their families while respecting individual differences and the need for confidentiality. Social Work students will be expected to collaborate with their Preceptor to create learning goals for their practicum. Students will be supervised in providing therapeutic intervention to clients and families in need of assistance to cope with social and emotional concerns; evaluate psycho-social needs of clients and their families; plan, implement, and evaluate the means of therapeutic intervention, resource coordination, and advocacy for families/clients; practice the maintenance of appropriate records and documentation; maintain communication with other team members regarding client care; take part in education and orientation as available; and participate in other activities that may align with their learning goals. Students will be supervised while engaging in the following activities: 1. Assessing the psycho-social needs of clients and their families. 2. Planning, implementing and evaluating the therapeutic intervention for families/clients. 3. Providing therapeutic intervention, as well as resource coordination and advocacy support to clients and families in need of assistance to cope with social and emotional, and practical concerns. 4. Providing direct and indirect client care in accordance with the treatment plan by using group and individual settings, collaborating with other professionals and other agencies as appropriate. 5. Practicing accurate documentation in accordance with established Centre procedures and meet the standards defined by the Ontario College of Social Workers and Social Service Workers.

Driver’s License and own car, or access to reliable means of transportation. Must complete centre orientation prior to or on start date of Practicum.
