Split placement between Inpatient Mental health unit at Oshawa site and Inpatient Medicine unit at Port Perry sites.

Description of the practicum site and student activities:
Student will be with the hospital social worker who is responsible for the provision of social work interventions/services in the inpatient units of acute care hospital.
Practicum Requirements:
☒ Immunizations per PHA Guidelines
☒ N95 Respirator/Mask Fitting
☒ Covid Vaccinations (Two doses of an approved Covid-19 Vaccination or one dose of Current Covid Vaccine if received after October 2023)
☐ Influenza Vaccination (Flu shot) (NOT MANDATORY BUT ENCOURAGED)
☒ Police Reference/Vulnerable Sector Check
☒ Comfort working with the population being served
☒ Comprehensive report writing and documentation

Mandatory Requirements:
- Two doses of Measles Mumps Rubella Vaccination or blood work showing immune status to Measles Mumps and Rubella
- Two doses of Varicella (Chicken pox) Vaccination or blood work showing immune status
- Vaccination with an Adult dose of Tetanus/Diphtheria/Pertussis (due every 10 years)
- Two step TB testing (if the test is positive, please contact Academic Affairs for further instruction)
- Blood work showing immune status for Hepatitis B
- Two doses of an approved Covid-19 Vaccination or one dose of Current Covid Vaccine if received after October 2023.
Tuberculosis (TB) Status: Affiliates are required to have 2-Step TB testing that can be performed by the Occupational Health Department as per policy. If 2-Step TB skin testing was previously completed, a single 1- Step TB skin test is required prior to starting. See page 2 for information if the Healthcare Provider wishes to complete.
Documentation of immune status (either “immune” or “not immune” as defined above), may be required by Occupational Health in the event of a potential exposure to disease
