University Health Network UHN, Toronto Rehab Institute (TRI)
Location: Toronto Rehab – University Centre (550 University Ave, Toronto) (in-person) and cross-site work across UHN with Social Work Discipline Head (in-person and remote)
Description of the practicum site and student activities:
This practicum will be split between on-site direct activities within the neurological rehab program at Toronto Rehab (TRI) as well as indirect and leadership projects within UHN where you will work closely with the UHN Social Work Discipline Head.
For the direct, clinical role at Toronto Rehab (approx. 2 days per week), you will work primarily on an inpatient rehab unit to support medically complex patients and families as they adjust to living with brain injury. You will have the opportunity to develop skills in completing comprehensive assessments, facilitating family meetings, planning complex discharges, attending team rounds, and linking patients and families to community resources. You will learn about living with brain injury and how to support patients and families in coping with challenges with cognition, movement, balance, coordination, speech and language, or behavioural and emotional issues.
For the indirect/leadership component of the practicum (approx. 1 day per week), you will have the opportunity to support UHN’s Social Work Discipline Head in the professional practice department. You will work with various leadership projects, quality improvement initiatives, and representing social work within the hospital system. A particular focus will be supporting quality improvement and capacity-building within palliative care.
Other opportunities available in professional practice include resource development, drafting of patient education materials, participating in committees and working groups, and aiding in developing practice standards.
