Description of the practicum site and student activities:

The student will be working on a multi-disciplinary medical model team with a Family Health Team (FHT) at St. Michael’s Hospital which is part of the Unity Health Toronto.

Students are required to deliver short term clinical and case management services to a diverse community of patients dealing with a wide variety of life issues, stress, anxiety, depression, grief/loss, trauma, life transitions etc., and connecting patients to outside resources as appropriate. The student will also get experience co-facilitating a group at the FHT according to population need and student interest. Students with a background in research will be welcomed to assist on any current FHT projects.
