Description of the practicum site and student activities:

The Family Health Team is a multidisciplinary team of family physicians, nurses, dieticians, pharmacists, and other allied health professionals. This placement is with the social worker in the Substance Use Management (SUM) program. Referrals come from family doctors and require students to contact patients, complete biopsychosocial assessments, and short-term counselling. Students are also expected to support with case management needs and updating resources for staff and patients. You have the option of facilitating psychosocial groups. Students are required to be in person at 73 Regent Park Blvd on Wednesdays, and 80 Bond on Thursdays.

Practicum Requirements:

☒ Immunizations per PHA Guidelines

☒ N95 Respirator/Mask Fitting

☒  Covid Vaccinations

☒ Influenza Vaccination (Flu shot)

☒ Travel by public transportation between home visits, or satellite offices, or community meetings, etc.

☒ Comfort working with the population being served

☒ Comprehensive report writing and documentation

☒ Lived experience/Current member of the community being served

Does your practicum require any previous experience?

Previous experience working with vulnerable clients. Previous experience working in harm reduction and substance use management is an asset.

Is this practicum in-person, remote or hybrid?

In person, with ability to work remotely

Will this practicum involve direct work or a mix of direct and indirect?

Mostly direct work with some indirect projects.
