Holland Bloorview Kids Rehabilitation Hospital Outpatient Social Worker, LIFEspan, Neuromotor, Community Life Skills and Collaborative Practice

Description of the practicum site and student activities:

The successful candidate will support multiple outpatient units and collaborative practice at Holland Bloorview Kids Rehabilitation Hospital. In this placement, the Social Work student will contribute to multiple interprofessional teams to support clients and families navigating life with disabilities. The student will join Holland Bloorview’s LIFEspan and Neuromotor clinics providing direct clinical Social Work to children and youth with disabilities. This will include system navigation, direct client/family support and the potential for short-term counselling. There will also be opportunities to support community life skill programming on an interprofessional team. Additionally, the student will work with our Collaborative Practice Leader to develop and execute leadership opportunities around Social Work practice in a healthcare setting. This fully immersive placement will appeal to students who are highly motivated, self-directed learners, and have strong interpersonal and communication skills.

Hybrid – students are always welcome on-site, however, there will be days where they can work from home.

Please indicate whether this practicum will involve direct work, indirect work or both.

Direct work – students will work directly with clients and families in different clinics and programs. They will also have the opportunity to build indirect skill with their time supporting collaborative practice.
